Fundamentally, I think organizations too often neglect to ask the simplest and most basic questions about the ways they communicate with their audiences:
Whose attitudes and behaviors are we trying to change?
Among that group, who (really) has heard what we have to say?
How are people who have heard us different from people who haven't heard us?
